Comments:
A complaint inspection was conducted on 07/13/2023 in response to a complaint received on 06/29/2023 regarding the care f infants. The inspector arrived at 2:00 PM and departed at 3:37 PM. The investigation included staff interviews, observation and review of documents. The inspector reviewed compliance in the areas of administration, staffing and supervision and programming.

Information gathered during the investigation determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-340-A
Complaint related: Yes
Description: Based upon observation and staff interview, the facility has not ensured that when staff are supervising children, they always ensure their care and protection.
Evidence:
1. Both staff 3 and staff 4 stated during interview that if a child in their mobile infant classroom is upset or crying and it is not during the rest period of 11:30 AM and 1:30 PM, the infant is placed on their stomach on the large shredded foam pillows and staff pat their backs. The pillows are soft, allowing for an infant?s head to sink into the pillow, a potential suffocation hazard. Staff stated the pillows are not sanitized between use, nor are they individually assigned with pillow covers, thus presenting a possible cross contamination issue.
2. Staff 3 stated and staff 4 witnessed that on 5/8/2023, staff 1 placed child 1, 13 months old, on the child?s stomach on the large shredded foam pillow so that staff 3 could pat the child?s back.

Plan of Correction: The facility responded with the following:
The pillows were removed from the classroom the day of the investigation visit. They have been in our Mobile Infant room for over five years with no issues or violations. They were used as part of the classroom calm-down corner for children to use as a comfort item. The children in the discussed room are over a year old and completely mobile and able to control their head on a pillow. If children were sleepy, outside of nap time, they were able to take short catnaps on the pillows. In the case a child did fall asleep on the pillow, a teacher was required to sit next to them, record the sleep in the parent app and conduct 5-minute sleep checks that note if the child was still asleep at that time and what their sleep position was. A child was not left unattended while asleep on a pillow.

Description: Based upon observation and staff interview, the facility has not ensured that pillows are not used by children under two years of age.
Evidence:
1. There were two 24? x 24? shredded foam filled pillows observed on the floor in the corner of the mobile infant classroom where staff 1 stated that the children in the classroom are 12 to 18 months of age.
2. Staff 3 and 4 stated that infants are placed on these pillows if they show signs of being sleepy or need to be soothed.

Plan of Correction: The facility responded with the following:
