Comments:
An unannounced complaint inspection was conducted on 07/24/2023. There were 6 children, ages 22 months-6 years, and 3 staff members present during the inspection. The inspector observed the following: free choice activities, A total of 2 children?s records and 2 staff records, were reviewed. The preponderance of evidence of supervision was not supported and complaint is not valid. The inspector arrived at 7:45 PM and departed at 8:20 PM.

Based on review of two staff records, the center failed to obtain the finding from a search of the central registry within 30 days of hire.

Evidence:

Staff # 1 had a documented employment date of 11/1/2021. A return search of the central registry was dated 06/28/2023. Staff # 3 did not provide documentation when requested.

Based on record review and interviews, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance.

Evidence:

1. The record for Child # 1 contained documentation that the first date of attendance for Child # 1 was 02/21/2022; there was no documentation of a physical exam on the day of the inspection.

2. The record for Child # 2 contained documentation that the first date of attendance for Child # 2 was 03/20/2020; there was no documentation of a physical exam on the day of the inspection.

Evidence:

1.Staff # 2 was observed working in the evening shift classroom as a program leader on the day of the inspection. After review of Staff # 2 record, there was no documentation of the educational requirements for a program leader position.

Description: Based on observation and interviews with staff, the center failed to ensure that for evening care; cots with at least one inch of dense padding shall be used by children who sleep longer than two hours.

Evidence:
Observed only cots in the sleeping area for the 6 children that rest during evening care. Confirmed by staff # 3 that the extra one-inch dense padding is not being used.
