Comments:

An unannounced complaint inspection was conducted on 07/24/2023. There were 6 children, ages 22 months-6 years, and 3 staff members present during the inspection. The inspector observed the following: free choice activities, A total of 2 children?s records and 2 staff records, were reviewed. The preponderance of evidence of supervision was not supported and complaint is not valid. The inspector arrived at 7:45 PM and departed at 8:20 PM. The record for Child # 1 contained documentation that the first date of attendance for Child # 1 was 02/21/2022; there was no documentation of a physical exam on the day of the inspection. 2. Description:	Based on observation and interviews with staff, the center failed to ensure that for evening care; cots with at least one inch of dense padding shall be used by children who sleep longer than two hours.
