Discussed the accident report and the requirement to report serious injuries to the department.
- Comments:

An unannounced complaint inspection was conducted on 8/08/2023 in response to a complaint that was received by the licensing office on 8/03/2023. Interviews were conducted with staff and documents were reviewed relating to care and supervision. The information gathered during this investigation does not support the allegations, so the complaint is determined to be not valid.
The center was cited for one violation unrelated to the complaint.

Standard #: 8VAC20-780-80-B-2 Complaint related: No Description: Based on interview, the center failed to inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
Evidence:
1. Child #1 was injured at the center on 5/30/2023 and required outside medical attention. Staff stated the child's guardian informed them of the outside medical attention approximately 3-5 days after the injury.
2. Staff stated that they did not know they needed to report it.Plan of Correction: I will report serious injuries in the future.
