Comments:
An unannounced renewal inspection was conducted on August 9, 2023 from 11:00am - 1:30pm. There were a total of 35 children with 11 staff members supervising. Children were observed engaged in playing outside, riding trikes, group activities, reading books, handwashing routines, and eating lunch. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, nutrition, children and staff records, special care and emergencies. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the Violation Notice issued to the facility.

Violations:
Standard #: 8VAC20-780-160-C
Description: Based on record review, staff did not submit the results of a follow-up tuberculosis (TB) test or screening every 2 years from the date of the initial screening or testing.
Evidence:
1. Staff #3 (DOH 3/5/09) most recent TB screening is dated 8/2/21(expired 8/2/23).
2. Staff #6 (DOH 10/2/07) most recent TB screening is dated 7/30/21 (expired 7/30/23).
3. Staff #7 (DOH 8/21/27) most recent TB screening is dated 8/6/21 (expired 8/6/23).

Plan of Correction: I will tell the staff they must bring a new TB test/screening results to me.

Standard #: 8VAC20-780-280-B
Description: Based on observation, potentially hazardous materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. The Primary A class (3-6 years) has cleaning products (Windex, Lysol, and Tile Cleaner) in an unlocked cabinet under the bathroom sink.
2. The Toddler class (ages 16-36 months) has cleaning product (Clean Freak) in an unlocked cabinet in the bathroom.
3. The E class (ages 6-12) has cleaning products (Lysol, Clorox cleaning wipes and Disinfectant spray) in an unlocked cabinet in the classroom (under the sink).

Plan of Correction: I will speak to the teachers and remind them to keep the products in a locked cabinet.

Standard #: 8VAC20-780-330-B
Description: Based on observation, resilient surfacing does not comply with the minimum safety standards.
Evidence:
1. The rubber mulch does not provide a safe fall zone around the large metal bus. There is less than 1/2" of rubber mulch around the bus.
2. Several large tree roots are exposed where the rubber mulch does not cover sufficiently There is less than 1" of mulch around the roots.

Plan of Correction: The construction and heavy rain washed a lot of our mulch away. We will have more mulch installed around the areas.

Standard #: 8VAC20-780-550-D
Description: Based on record review and interview, the center did not implement a monthly practice evacuation drill.
Evidence:
1. The evacuation log sheet did not have a July practice drill documented.
2. The Director acknowledged the evacuation drill was not practiced in July.

Plan of Correction: I will ensure we practice the drill each month going forward.

Standard #: 8VAC20-780-550-I
Description: Based on observation, a 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center was not posted in a visible place at each telephone.
Evidence: Emergency numbers were not posted near any of the telephones in the center.

Plan of Correction: I will post the numbers today.
