Technical Assistance:
Discussed with the Provider: sanitation inspection, mixed-age groups, and building official.

Comments:
An unannounced monitoring inspection was conducted on 08/10/2023 from 10:30 am-12:55 pm. There were 81 children supervised by 12 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, and policies were inspected. Children were observed participating in group play and tabletop activities. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations:
Standard #: 8VAC20-780-350-C
Description: Based on review of attendance records and inteview, the center exceeded the 1:5 staff-to-children ratio requirement for the 18 months-3-year-old mixed-aged classroom.

Evidence:

1. In the toddler classroom, there were 2 children in care who were 18 months old and the other 24 children were over the age of 18 months old.

2. Since the children aged 18 months were in care the ratio required is 1 staff to 5 children.

3. There was 4 staff with 26 children during the inspection.

4. The ratio of the youngest age must be followed in a mixed-age grouping such as this.

Plan of Correction: Teacher was out sick, put a teacher in the classroom to meet the ratio.

Standard #: 8VAC20-780-510-G
Description: Based on medication review and interview, medication was not labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.

Evidence: Child #2 has emergency medication that is missing a prescription medication label that includes the child's name, the name of the medication, the dosage amount, and the time or times to be given.

Plan of Correction: Medication has label with the child's name, the name of the medication, the dosage amount and times to give.

Standard #: 8VAC20-780-560-G
Description: Based on observation and interview, food brought from home was not dated.

Evidence:
1. In the Pre-K/Kindergarten Classroom, 10 out of 10 lunch boxes were not dated.
2. In the Pre-Primary Classroom, 7 out of 7 lunch boxes were not dated.

Plan of Correction: Dated the food that was brought from home.
