A monitoring inspection was conducted in response to a self-reported incident that was received in the licensing office on 7/21/2023 regarding behavioral guidance and care/protection of children. On 7/24/2023 an on-site investigation was conducted. Interviews were conducted, records were reviewed, and observations were made.
