Comments:
An unannounced monitoring visit and facility tour was conducted on 08/10/2023. At the time of inspectors' arrival there were 59 infant to preschool aged children in care with 10 staff members and 2 administrators. Children were observed interacting with staff, nap, and free play. A sample of 5 children's records and 3 staff records were reviewed. Injury reports were also reviewed. Director reported medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Violations:
Standard #: 8VAC20-780-60-A-8
Description: Based on record review and interview it was determined the center did not ensure that a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction was in their record.
Evidence:
1. The record child #1(enrolled 4/24/23) did not contain a written allergy care plan, for their documented allergy.
2. Director confirmed this plan was not available during this inspection.

Plan of Correction: Will request allergy action plan from parent.

Standard #: 8VAC20-780-500-A
Description: Based on observation it was determined the center did not ensure that staff washed their hands and the hands of the child after diaper changes.
Evidence:
1. Inspector observed staff #1(hired 5/15/23) change the diaper of an infant with one gloved hand, after changing child staff removed glove and picked up the child and placed the child in a stationary activity center without washing their hands or the hands of the child.

Plan of Correction: Staff will be retrained at monthly staff meeting.

Standard #: 8VAC20-780-570-A
Description: Based on observation and interview it was determined the center staff did not ensure that when children re placed in a high chair, the protective belt is securely fastened.
Evidence:
1. Inspector observed three children in the infant room seated in high chairs without the protective belts fastened.
2. Staff confirmed the protective belts were not fastened.

Plan of Correction: Staff will be retrained at monthly staff meeting.
