Comments:
An unannounced monitoring inspection was conducted on 8/14/2023 from 9:17am to 11:25am. At the time of entrance,59 children were in care with 12 staff members present. Children were observed washing hands, being held, eating breakfast, getting ready for waterplay, circle time listening to the teacher read a books and playing outside, Infants were observed sleeping in cribs, being fed a bottle and playing on the floor. A selection of staff and children records, medications, the physical space, evacuation drills, allergy action plans and attendance records were reviewed. The site was clean, organized and contained an abundant supply of developmentally appropriate materials.. Interactions between the children and staff were positive. Standard #: 8VAC20-780-270-A
Description: Repeat Violation

Based on observation, the areas and equipment inside and outside were not main tained in a safe and operable condition.
Evidence:
1. In the Two's B classroom, the paint was chipping near 3 windows. A low shelf was peeling off/cracking on top of the shelf. A sink that children use had a large crack near the faucet approximately 8 inches long.
2. In the Toddler classroom, a blue sofa had a rip in the fabric approximately 1 inch on top.
3. On the playground, a small ride on toy was splintering at the bottom base. The door strips were falling off the red door and the paint was peeling to the right of the door and bottom left of the door.

Plan of Correction: Management has placed a work order to have the center painted in areas of
need. Equipment that is not in good condition has been removed from
inside and outside of the center.

Description: Repeat Violation

Based on observation, hazardous substances were not kept in a unlocked place using a safe locking method that prevent access by children.
Evidence:
1. In the children's bathroom,3 restroom cleaning sprays and 1disinfectant sanitizer were inside an unlocked cabinet.

Plan of Correction: All hazardous substances have been placed in cabinets with proper working
locks. Management have met with and completed the Hazard Communication
Program training with all staff members. Classroom cleaning guides have been
reviewed and reposted. Staff have acknowledged that all chemicals
must be locked and inaccessible to children at all times.

Standard #: 8VAC20-780-500-B
Description: Based on interview and observation, the center did not meet all diapering requirements.
Evidence:
1. The staff stated they dispose of diapers into the trash can near the changing table, but it was not foot-operated or used in such a way that neither the staff
member's hand nor the soiled diaper touches an exterior surface of the storage
system during disposal.

Plan of Correction: Equipment has been replaced to meet all diaper changing requirments.
Management has met with all staff to review diaper changing best practices
