A Subsidy Health and Safety inspection was conducted on 8/15/2023 to review supplemental health and safety requirements. There were 63 children present with 12 staff supervising.
A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 8VAC20-790-600-F Description: Based on a review of six staff records and interview, the vendor did not ensure that four staff annually completed the department's health and safety update course as required.
Evidence:
1. The record of staff #1 (date of hire 5/11/2018) did not contain a 2022 or 2023 annual health and safety update certificate. The record of staff #3 (date of employment: 3/7/2022) contained a health and safety update dated 3/2/2022. The record of staff #4 (date of employment: 11/6/2019) contained a health and safety update dated 3/13/2022. The record of staff #6 (date of employment: 8/17/2017) did not contain a 2022 or 2023 annual health and safety update certificate.
2. The vendor acknowledged that the certificates were not in the records and believed the most recently required annual updates to not have been completed by the 4 staff.Plan of Correction: Not available online. Contact Inspector for more information.
