A monitoring inspection was completed at the center on August 8, 2023 from 1:00 pm until 3:47 pm. There were 51 children present, ages to infancy to school age with 9 staff supervising including administrative staff. The inspector completed observations of the children in care napping, completing bathroom breaks, watching a movie in classroom, and completing tummy time, asleep in age appropriate equipment, and interacting with staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 5 children's records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22.1-289.035-B-4 Description: Based on review of record and interview, the center did not ensure to obtain a copy of results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence: In Staff#1's record (date of hire: 05/03/2023) there were no results of an out of state central registry search found. Interview with staff confirmed the results were not available for review during the inspection.Plan of Correction: The center's director will submit request for the out of state central registry search for staff and include results in file once completed.
Standard #: 8VAC20-780-270-A Description: Based on observations and photographs, the center did not ensure all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.
Evidence:
1. There was observation of a yellowish substance on the floor surrounding the toilet area and on the toilet seat.
2. There were multiple areas of peeling paint on the back wall in the infant room.
3. There were multiple areas of peeling paint on the diapering station in the infant room.
4. Photographs taken of these observations during the inspection.Plan of Correction: The center Director will submit maintenance work order for all areas today and request completion by 8/12/2023
Standard #: 8VAC20-780-500-B Description: Based on observations, interview and photographs, the center did not ensure the diapering area meets all requirements.
Evidence:
In the Infant room, there were two diapering pad with multiple torn areas causing an absorbent surface.
Interview with staff confirmed use of the diapering stations in the room.
Photograph of these observations during the inspection.Plan of Correction: The center Director will order new diapering pads today.
