Comments:
An initial inspection was initiated on 8/10/2023 and concluded on 8/10/2023. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of one staff record was reviewed. The inspection started at 9:45 am and concluded at 11:45 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1. The roof covering the back porch, to be used by the children, has nails exposed. There are several areas of rotten wood under the roof.
2. The cinderblock building behind the center has a down spout with sharp edges. This is located in the outside play area for the children.
3. The side area to be used as a playground area has cinderblocks, pieces of 4x4's, scrap wood and trash strown about.
4. The infant room has an area with wooden 4x4's that are splintered.
5. In the toddler and Pre-K areas are shelves on top of the cubbies, not attached to the wall.

Plan of Correction: The owner/director will ensure that the exposed nails are cut off. The wood will be replaced. Sharp edges will be covered. All trash, cinderblocks, trash will be removed from the playground area. The wooden 4x4's will be covered to ensure that no splinters are accessible by the children.

Standard #: 8VAC20-780-500-B
Description: Based on observation the center failed to ensure that the diapering area shall have the following, a sink with running warm water not to exceed 120 degrees F.
Evidence:
The sinks have a water temperature of 136 and 125 degrees F.

Plan of Correction: The owner/director will ensure that the water temperature is lowered and maintained at or lower than 120 degrees F.
