Comments:
An unannounced complaint investigation was initiated on 8/4/23 in response to a complaint received in the licensing office regarding allegations of infants being fed table food and whole milk without parent permission, and being left in their cribs for extended periods of time when they were not asleep. Interviews were conducted, a classroom observation was conducted, and two staff records were reviewed. The allegation regarding infants being fed table food and whole milk without parent permission was determined to be valid and a violation was cited. The allegation regarding infants left in cribs for an extended time was unable to be substantiated due to insufficient corroborating evidence. An exit interview was conducted with the Director on 8/16/2023. Description: Based on interviews conducted, the center did not ensure that infants were fed in accordance with parental instructions. Evidence: Interviews revealed that in approximately March 2023 an eight-month-old infant was fed ?sloppy joes? when the parents had not given permission for the infant to eat that food. The same infant was given whole milk in approximately June 2023 without parent permission.

Plan of Correction: All infant staff have been trained to follow food forms that are given to parents to fill out along with a menu attached to the form. These forms are placed on each child's clipboard so the teacher can clearly see what a child can and can't have for food and milk.
