Comments:
An unannounced monitoring inspection was initiated and completed on 8/14/2023. The inspector was on site from 9:50am until 12:30pm.

There were 65 children, ages 3 months to 10 years old under the supervision of 10 staff. Children were observed during indoor free play and lunchtime. Five children's records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289-036-B-4
Description: Based on record review, the center failed to obtain a copy of a criminal record check and a child abuse and neglect registry check from any state in which the individual has resided in the preceding five years.

Evidence:
1.The record for Staff 1 contained documentation that Staff 1 has resided outside of the state within the preceding five years. There was not a child abuse and neglect search registry check from the state in which Staff 1 resided within the preceding five years available for review.
2.The record for Staff 1 contained documentation that Staff 1 has resided outside of the state within the preceding five years. There was not an Out of state Sex Offender Registry completed for each state an individual has resided in the past 5 years.

Description: Based on record review, the center failed to obtain a copy of the child physical examination before child?s first day or within 30 days after first day of attendance.

Evidence:
Child #2, which was enrolled on 12/5/2022 had no documentation of a physical on file during the inspection on 8/14/2023.

Description: Based on record review, the center failed to keep a separate record foreach child which shall contain written agreements between the parent and the center.

Evidence:
1.Child #3, had an enrollment date of 6/2/2023 had no proof of ID on file during the inspection on 8/14/2023.
2.Child #4, had an enrollment date of 7/24/2023 had no proof of ID on file during the inspection on 8/14/2023.
3.Child #5, had an enrollment date of 6/21/2023 had no proof of ID on file during the inspection on 8/14/2023.

Description: Based on observation, the center failed to maintain areas and equipment of the center in a clean, safe operable condition.

Evidence:
In the indoor playroom, the paint in the window seal has become chipped and the wood in the seal has become splintered. This is a hazard to children.
