Comments:
Today an unannounced investigation was conducted regarding a self-reported incident that was reported to the licensing office on 7/14/2023. This visit was conducted between 1:45pm and 3:00pm. Interviews were conducted and a review of video footage provided by the center was observed. After carefully interviewing all involved parties, violations were cited and can be reviewed on the Violation Notice.

Violations:
Standard #: 8VAC20-780-40-E
Description: Repeat Violation.
Based on a review of the centers policies and interviews, the operational responsibilities of the licensee did not ensure that the centers activities and services are maintained in compliance with these standards, the centers own policies and procedures that are required by these standards.
Evidence:
1. On 7/13/2023, Staff #1 pickup up Child #1 (6 years old) and placed them on their hip, took a few steps and then sat down to discuss an altercation that Child #1 was involved in.
2. Staff #1 stated that they are trained not to pick up children however they felt as though Child #2 was in danger and they ?made a split decision? to pick-up Child #1 and try to mitigate in another area?
3. The program director confirmed that the center policy would be to not pick-up the child.

Plan of Correction: Staff should have asked for assistance from another staff member that was in the room.

Staff have been retrained at our monthly staff meetings and we?ve had all the staff sign our staff handbook and when the new preschool program starts in a few weeks we?ll be discussing alternative solutions again.

Staff are also now using behavior notebooks to document challenging behavior to ensure that we have open communication with parents.

Description: Based on a review of video footage obtained from the center and interviews, behavioral guidance was not age and stage appropriate.
Evidence: On 7/13/2023, Staff #1 picked up Child #1 (6 years old) and placed them on their hip to walk away from an altercation with Child #2. Picking up Child #1 was not age and stage appropriate.

Plan of Correction: Staff should have asked for assistance from another staff member that was in the room.
