Comments:
Today an unannounced Renewal Inspection took place between the hours of 11:50am and 1:30pm. There were 3 classrooms observed with a total of 19 children with 6 staff within the supervision guidelines. The children were observed eating lunch and transitioning to nap. A complete inspection of the physical plant, children and staff records, fire drill log and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations:
Standard #: 22.1-289.036-B-2
Description: Based on interview, each agent of the legal entity did not submit to fingerprinting.
Evidence: Staff #4 confirmed that agent #1 has not completed fingerprinting.

Plan of Correction: We will have [agent #1] complete fingerprinting.

Standard #: 8VAC20-780-130-E
Description: Based on record review, the center did not obtain documentation of updated immunizations once every six months for children under the age of two years.
Evidence: Child #1 (21 months old) most recent documented immunizations are dated from when the child was 6 months old.

Plan of Correction: We have requested for the parents to bring the updated form.

Standard #: 8VAC20-780-280-B
Description: Repeat Violation.
Based on observation, hazardous substances were not kept in a locked place using a safe locking method.
Evidence: A spray bottle with a bleach/water solution was observed on the open shelf above the preschool changing station.

Plan of Correction: I will retrain the staff.
