Technical Assistance:
Technical Assistance given and resilient surfacing licensing standards were discussed.

Comments:
An unannounced renewal inspection was conducted on August 17, 2023. The center closed temporarily from October 14, 2022 and reopened on January 24, 2023. The inspector arrived at 10:30AM and departed at approximately 12:40PM. There were 25 children present and 7 staff. The Inspector reviewed five children?s records and six staff records. The Inspectors observed staff and children?s interactions throughout the inspection. Children were observed during free play, lunch, group activity, diapering, and nap. No transportation is provided at this center. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the center?s director at the exit interview.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, it was determined that the center did not ensure that the areas and equipment of the center be maintained in a safe and operable condition.
Evidence:
1. In the 2 year old and preschool classrooms, there is a shelf in each room that is unstable and poses as a tipping hazard to the children in care.
2. In the 2 year old and preschool room, there are cracked and broken toy bins which pose as pinching hazards to the children in care.
3. Staff #1 confirmed that the shelf is unstable and poses as a tipping hazard.

Plan of Correction: The center responded with the following: Staff will put in a work order to secure and bolt the shelf to the wall. The toy bins will be disposed of and replaced.

Standard #: 8VAC20-780-440-I
Description: Based on observation and interview, it was determined that the center did not ensure that cribs shall be placed where objects outside the crib such as cords from blinds or curtains are not within reach of infants or toddlers.
Evidence:
1. There is a crib parallel and beside the window in which the child that is assigned the crib can reach the venetian blinds. This poses as an injury hazard should the child reach for the blinds.
2. Infant staff confirmed that the venetian blinds are in reach of the child assigned to the crib located by the window.

Plan of Correction: The center responded with the following: A work order will be put in to remove the blinds from the window.

Standard #: 8VAC20-780-440-J
Description: Based on observation and interview, it was determined that the center did not ensure that there shall be at least thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib.
Evidence:
1. Three of the cribs in the infant room, which were occupied during the inspection, did not have the required thirty inches of space between service sides of occupied cribs and the other furniture.
2. Infant staff and the Director confirmed that there was not thirty inches of space between service sies of occupied ribs and the other furniture.

Plan of Correction: The center responded with the following: The cribs will be rearranged to ensure that there is the appropriate spacing for the service side. The center will be looking to relocate the infant room as there is a larger room at the center.

Standard #: 8VAC20-780-500-B
Description: Based on observation it was determined that the center did not ensure that the diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use.
Evidence:
1. During observation of a diaper change in the 2 year old classroom, the 2 year old teacher did not clean and sanitize the diapering surface after changing a child?s diaper.

Plan of Correction: The center responded with the following: Staff will be reminded to ensure that the changing table will be clean and sanitized after every use.
