Comments:
An inspection was conducted in response to a self report received by the licensing office regarding supervision concerns. An inspection was initiated on July 19, 2023 beginning at 11:30 a.m. and ended at 2:00 p.m. with the director.

Staff interviews were conducted. Documentation including a staff record and child's record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-340-C
Description: Based on staff interviews, the center failed to ensure that during field trips when one or more children were present there was one staff member and an immediately available staff member, volunteer or other employee with a direct means of communication between the two of them. Evidence: On July 11, 2023, Staff #1 was alone with a group of 7 children ages 14 to 17 years old at the National Museum of the Marine Corps. Staff interview revealed that there was not a direct means of communication between Staff #1 and the driver as the walkie talkie used by the center was out of range and the driver did not have their cell phone with them during the trip. Further interview revealed that the driver was not on site during the field trip and left the premises to pick up another group of children and staff.

Standard #: 8VAC20-780-340-G
Description: Based on staff interviews, it was determined that children 10 years of age and older were not within actual sight and sound supervision as required. Evidence: On July 11, 2023, at approximately 3:10 p.m., Child #1, age 14 years old, was not within sight and sound supervision for approximately 20 to 30 minutes when left at the National Museum of the Marine Corps on a field trip.

Description: Based on staff interviews, it was determined that when the center provided transportation, they were not responsible for all children in their care from the time children boarded the vehicle until returned to the parents/guardian as required.
Evidence:
On July 11, 2023, Staff #1 did not account for all children in care at the end of a field trip and left Child #1, age 14 years old, alone at the National Museum of the Marine Corps.
