Comments:
A monitoring inspection was initiated on August 15, 2023 and concluded on August 15, 2023. There were 13 children present, ranging in ages from 18 months to 5 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.

Please complete the plan of correction and date to be corrected sections for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s): 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. The Preschool classroom had an unlocked cabinet containing Febreze and Krylon Lacquer. The label on the Febreze stated "keep out of the reach of children". The label on the Krylon Lacquer stated, " keep out of the reach of children", and "DANGER! EXTREMELY FLAMMABLE."
2. In the Toddler Room, GooGone was observed on top of a cabinet close to the file cabinet out of the reach of children but not in a locked place using a safe locking method. The label on the GooGone stated "DANGER!" and "keep out of reach of children and pets".

Plan of Correction: Teachers have removed or secured all hazardous materials that were found in classrooms. Rooms will be checked after the building is used by other groups/after the weekend. Workers/visitors doing general repairs/maintenance will be reminded to take all tools/materials with them, and teachers will conduct a check of the area and remove and properly dispose of anything left behind.

Standard #: 8VAC20-780-520-A
Description: Based on observation, the center failed to ensure that all over-the-skin products shall not be kept beyond the expiration date.

Evidence:
1. In the Toddler Room, in a locked cabinet, there was a tube of Desitin diaper cream that expired in February 2023.
2. Staff 2 confirmed the expired diaper cream.

Plan of Correction: All expired over-the-counter skin products have been removed and properly disposed of. The teacher in charge of the toddler room will keep note on over-the-counter skin products that are about to expire and the Director will review this list on a monthly basis.
