Comments:
A non-mandated monitoring inspection was conducted on August 17, 2023 from 11:00 A.M.-1:35 P.M. There were 45 children present, ranging in ages from three months to eleven years of age, with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of five child records and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of staff records, the center failed to ensure that a tuberculosis screening be completed at the time of employment and prior to coming into contact with children, or within the last 30 calendar days of the date of employment.

Evidence:
1. Staff #3's date of hire was 06/26/2023. There were two tuberculosis screening on file one dated 05/17/2023 and another dated 07/17/2023.
2. Administration verified the date of employment for Staff #3, and that they had been in contact with children since 06/26/2023.

Plan of Correction: We plan to have tuberculosis screening completed within the correct timeframe of employment for future employees. That will be within 30 calendar days of prior to the date of employment.

Standard #: 8VAC20-780-60-A
Description: Based on a review of children records, the center failed to ensure that all required information was in the record.

Evidence:
1. Child #1's record did not contain the address for the two designated emergency contacts listed, and any allergies and intolerance to medication or any other substances, and actions to be taken in an emergency situation.
2. Child #2's record only contained one of the two required designated people to call in an emergency if a parent cannot be reached, and any allergies and intolerances to medication or any other substances, and actions to be taken in an emergency situation.
3. Administration verified the records were missing all the required information.

Plan of Correction: On 08/17/2023, Child #1's registration record was changed to the proper form. Our facility plans to use the DOE registration forms in the future. Those forms contain the correct number of emergency contacts required.

Standard #: 8VAC20-780-560-L
Description: Based on observation, the center failed to ensure that staff shall sit with children during meal times.

Evidence:
On 08/17/2023 the Licensing Inspector observed Staff #1 supervising four children in the Two's Classroom. The four children were sitting at the small table in the middle of the classroom eating lunch. Staff #1 was observed standing in the back of the room behind the changing table talking to Staff in the Three/Four Classroom for approximately five minutes. Staff #1 then walked back in forth in the back part of the classroom, and did not sit with the children at meal time.

Plan of Correction: Our staff was reminded that the standards states that children must be sitting during any meal. The staff were reminded to sit with children during meals in the future.
