Discussed recent interior painting and that required postings and diaper changing areas be returned to operational status as soon as possible.
Discussed and emailed the center director information about requesting a sanitation inspection that is required by all licensed Child Day Centers.
Discussed requirements for medication authorization forms to include dates of effect, and use of Fairfax County Public School authorization forms.
Discussed water temperature of the foot operated sinks and emailed the center information on requesting an allowable variance.
Recommended center administration conduct an annual review of all children's files that is signed off by the parents and guardians.
Discussed the temperature of the slow cooker used in the infant room to warm bottles and safe handling of children while bottles are placed in, and removed from the warmer. Recommended securing warmer so that the slow cooker cannot be jostled or fall from it's location.
Recommended removing rolling desk chair from the infant room to minimize fall risks of staff while holding infants.
Discussed USDA guidelines for feeding infants flavored milk.

An unannounced monitoring inspection was conducted today from 9:26am-12:00pm to determine compliance of the facility while they operate under a provisional license. There were 39 children directly supervised by 9 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in being bottle fed, building with blocks, doing puzzles, and playing outside. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-210-A Description: Based on record review, one classroom was operating without a staff member whose record demonstrates that they have met the requirements to operate as a program leader.
Evidence:
Staff 1, 2's records did not contain documentation to demonstrate that the staff members have fulfilled all requirements to be qualified as a program lead. Both staff were the only caregivers in the classroom during the inspection.Plan of Correction: 1)Staff will continue to receive
professional development and
training to meet requirements.
2)Staff listed as assistant teacher for
toddler 2 class has met training
requirements as 'lead' however, it is
her preference to remain an assistant.
3)Admin staff is responsible for
ensuring staff has/obtain qualifications
required to be lead teachers.
Standard #: 8VAC20-780-270-A Description: Based on observation, areas inside the center were not maintained in a safe and operable condition.
Evidence:
1. A folding door was observed off of it's hinges in the school age room.
2. A knife block containing chef's knives was observed on the kitchen counter. The door to the kitchen was unlocked and opens into a classroom with children present.Plan of Correction: 1) The kitchen door has been fixed and door will remain closed at all times.
2)Building maintenance will be
checked frequently. Documentation
and check lists will be kept.

any damages to supplies, furniture and
equipment. Admin is responsible for
making sure the items reported or
observed as broken are fixed/replaced.
Standard #: 8VAC20-780-280-B Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. A canister of disinfectant wipes was observed in an unlocked cabinet in the infant room.
2. Liquid sanitizer, laundry detergent, dishwashing detergent, disinfectant wipes, and a animal glue trap was observed in an unlocked cabinet in the kitchen.Plan of Correction: 1)Hazardous materials have been put 8/21/2023
away and locked away.
2)Every class has their designated
areas to store all cleaning chemicals.
They will be reminded to keep these
areas locked after using.
3)Staff are responsible for making sure
they are keeping hazardous
substances locked and out of reach
of children in their care.
Standard #: 8VAC20-780-290-A-3 Description: Based on observation, electrical outlets did not have protective covers installed.
Evidence:
An electrical outlet was observed without a protective cover in the Three's classroom.Plan of Correction: 1)All electrical outlets have been
covered right away.
2)Staff and Admin will regularily check
to make sure outlets are always covered.
3)Staff and Admin staff are responsible.
Standard #: 8VAC20-780-370-5 Description: Based on observation, staff failed to provided awake infants not playing on the floor or ground a change of play space every thirty minutes or more often as determined by the individual infant's needs.
Evidence:
Infants were observed occupying bouncy seats and swing in the classroom from the start of the inspection at 9:30am until at least 10:15am.Plan of Correction: 1)Staff will be reminded to change out
toys in play area every 30 minutes, and
to engage/interact with children.
2)An updated daily schedule will be
posted in the classroom to guide staff
and encourage changing of spaces.
3) Staff are responsible for making sure
the children are provided an interactive
and stimulating environment. Admin
assists staff with training/guidance.
Standard #: 8VAC20-780-430-B Description: Based on observation, materials used by children were not aga appropriate.
Evidence:
The toddler class was using small craft pom poms in a gross motor activity. The pom poms are not age appropriate for children under the age of three due to posing a choking risk.Plan of Correction: 1)The materials found will be removed
from the class immediately.
2)All staff will be reminded to conduct
a choke hazard test on all materials
and toys of their class.
3)Admin and staff are responsible
for making sure all toys and materials
are age appropriate for groups.
Standard #: 8VAC20-780-440-J Description: Based on observation, there was not thirty inches of space between the service side of occupied cribs.
Evidence:
An infant was observed sleeping in a crib with both sides of the crib within 11 inches of neighboring cribs.Plan of Correction: 1)Cribs not being utilized have been
removed temporarily to allow more
space.
2)Staff will be reminded to keep
required space between cribs.
3)Staff and Admin are responsible for
making sure space is available in
between cribs.
Standard #: 8VAC20-780-500-A Description: Based on observation, staff did not wash hands after diapering.
Evidence:
Staff 3 was observed to change a child's diaper and resume caregiver tasks without washing hands.Plan of Correction: 1)Staff will be reminded to wash hands
before and after all diapering.
2)Posters are available near handwashing
stations to serve as a reminder.
3)Staff are responsible for washing their
hands before and after all diapering.
Standard #: 8VAC20-780-530-A-1 Description: Based on record review and observation, at least one teacher in each room did not have a current certification in CPR as appropriate to the age of the children in care from an organization such as the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council that was obtained after an in-person competency demonstration.
Evidence:
On the date of inspection Staff 5 was the only teacher present in the classroom. Staff 5's CPR certification was obtained by completing an online course with no in-person competency componet.Plan of Correction: 1)Staff will be reminded that their
training is to be hybrid or in person only.
2)Admin will make sure the training
for CPR/First Aid is acceptable from a
credible source and in person. CPR/
First Aid training is provided on site for
current staff. Admin staff will review
new hires with current certification and
make sure it meet all requirements.
3)Admin staff is responsible for making
sure staff CPR/First Aid training is
up to date.
