Technical Assistance:
Technical assistance given and licensing standards were reviewed regarding the areas of children?s play equipment.

Comments:
An unannounced renewal inspection was completed August 23, 2023. Inspector arrived at 10:40am and departed at approximately 1:30pm. There were 83 children and 9 staff members present. The Inspector observed seven children?s records and seven staff/board member?s records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during indoor and outdoor play, group activity, lunch, diapering, handwashing, bottle feeding, and nap. First aid kits and emergency preparedness documents were observed. Medications are administered at the center and were observed. Transportation vans were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the Director at the exit interview.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, it was determined that the center did not ensure that the areas and equipment of the center outside shall be maintained in a safe and operable condition.
Evidence:
1. The large playset located on the playground has the following concerns:
a. Crack and rusted flooring where children walk, stand and sit on posing as an injury and tripping hazard;
b. The stairs have several cracked and peeling edges which poses as a pitching hazard to the children when climbing to the top of the playset apparatus;
c. Peeling pain in various areas of the poles and handles of the playset;
2. The play area is accessible to children during outdoor play and there are no cautionary or preventative measures to keep children off the unsafe areas of the playset apparatus.

Plan of Correction: The center responded with the following: The center stated that it is proposed to have the playset apparatus removed off the property. The center will speak with the corporate office to discuss the concerns. The center will ensure that the children continue to not use the playset apparatus.

Standard #: 8VAC20-780-510-L
Description: Based on observation and interview, it was determined that the center did not ensure that medication, except for those prescriptions designated otherwise by written physician's order, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In looking through the 3 year old classroom first aid kit, the kit was open in the safety bookbag with three packets of non-aspirin medication. The safety bookbag holding the first aid kit did not have a safe locking method to prevent access by children.
2. Staff #1 confirmed that there were three packets of non-aspirin medication that was not locked to prevent access by children.

Plan of Correction: The center responded with the following: The center appropriately disposed of the medication during the course of the inspection.

Standard #: 8VAC20-780-540-C
Description: Based on observation and interview it was determined that the center did not ensure that the required first aid kits shall include triangular bandages.
Evidence:
1. There were no triangular bandages observed in the first aid kits.
2. Staff #1 confirmed that there were no triangular bandages in the first aid kits in the center.

Plan of Correction: The center responded with the following: The center will purchase triangular bandages and place them in the first aid kits in the center.
