Comments:
An unannounced monitoring inspection was conducted today approximately 10:51 am to 12:40 pm.
There were 87 children directly supervised by 17 staff. The physical plant, 9 staff records, 6 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. No new medications since the last inspection. Children were observed participating in group play, reading books, playing outside, and transitioning to leave for a bowling field trip. There was an assortment of age appropriate materials for the children in care. Staff were engaged and interactive with the children.

Description: Based on records review, the center did not ensure each staff member submit documentation of a negative tuberculosis screening.
Evidence:
1. Staff #9 (5/02/2023) did not have documentation of the screening at the time of employment and prior to coming into contact with children.

Standard #: 8VAC20-780-160-C
Description: Based on staff records review, the center did ensure staff submitted documentation of a tuberculosis screening at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening.

Description: Based on children records review, 2 out of 6 records, the center did not obtain the required documentation in each child enrolled record.

2. Child #5 (date of enrollment 8/01/2018) did not have name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
