An unannounced monitoring inspection was initiated and completed on 08/24/2023. The inspector was on site from 12:35pm until 3:00pm.
There were 45 children, ages 5 months to 4 years old under the supervision of 9 staff. Children were observed during naptime and snack. Five children's records, one medical record and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

1.In the afterschool game room, the silver wire protecter is hanging from the wall. This is a entrapment hazard to children.

Standard #: 8VAC20-780-330-E Description: Based on observation, the center failed to ensure that sandboxes with bottoms which precent drainage shall be covered when not in use.

Standard #: 8VAC20-780-500-B Description: Based on interview, the center failed to ensure the diapering area shall have a nonabsorbent surface for diapering or changing. For children younger than three years, this surface shall be a changing table, or a countertop designed for changing.
Evidence:
When asked the diapering process for children under two, staff stated there is no changing table located in the building for the children to be changed.
