Comments:
A complaint was received in the licensing office on August 21, 2023 regarding mouse droppings in the center. An investigation was initiated on August 22, 2023. An on-site inspection was conducted on August 22, 2023 between the hours of approximately 8:15 a.m. and 9:20 a.m. Observations were made and interviews were conducted. The preponderance of evidence gathered during this investigation supports the allegation, therefore, the complaint is determined to be valid. Violations cited can be found on the violation notice. Description: REPEAT VIOLATION

Based on observation and staff interview, areas of the center were not maintained in a safe and clean condition.

Evidence:
1. Mouse droppings were observed in the following areas of the center.
a. In the Penguins classroom on a toy shelf by the emergency exit door, on a green chair by the cots, and on the floor by the trash can.
b. In the Ducks classroom, on two toy shelves by the windows, on three sets of cubbies, and on the floor in front of a cubby set.
c. In the Cubs classroom, at the bottom of the cubbies.
2. Staff #1 stated that they saw mouse droppings in the Ducks classroom and called pest control that morning.
3. Staff #2 stated that they saw mouse droppings in a classroom approximately 2 weeks ago.
4. Staff #3 stated that they have been aware of mouse problems for approximately six months and mouse traps have been placed in classrooms.
5. Staff #4 stated that a mouse was spotted in the first floor break area approximately two months ago.
6. On August 21, 2023, a staff member was observed sweeping up mouse droppings by Witness A.

Plan of Correction: We were made aware of this problem during a transition in June and immediately contracted a new Pest Control and cleaning company. All recommendations and structural changes needed to address the issue have been implemented, including increasing external dumpster pick-ups to twice a week. Protocols for proper daily sanitizing of classrooms have been reiterated to the staff.

A new pest control treatment was performed again on August 22, 2023, and will continue as required for childcare settings to ensure a safe and secure environment.
