Comments:
An unannounced renewal inspection was initiated on 8/22/2023. The inspector was on site from 2:55pm until 3:55pm. Additional information was received on 8/28/2023.

There were 45 children, ages 5 years to 11 years old under the supervision of 3 staff. Children were observed during indoor free play and snack time. Five children's records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on record review and staff interviews, the center failed to obtain documentation that each child has received immunizations required by the State Board of Health the child can attend the center.

Evidence:
1.Child 5, with an enrollment date of 08/14/2023 did not contain documentation that they received the immunizations required by the State Board of Health.

Plan of Correction: Administrator will be going to Goodview to correct and organize the children's files to ensure they are up to date and have all the required documents inside.

Standard #: 8VAC20-780-160-A
Description: Based on record review, the center failed to have documentation of the TB screening submitted at the time of employment and prior to coming into contact with children.

Evidence: Staff #3 which was hired on 8/16/2023 had no record of a TB screening on file during the inspection on 8/22/2023.

Plan of Correction: Staff will be having their TB test done next week and I will make sure it is updated in her file.

Standard #: 8VAC20-780-60-A
Description: REPEAT VIOLATION
Based record review, the center failed to ensure that the separate record for each enrolled child shall contain name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Evidence:
1.Child #1 had no address for either emergency contacts on record.
2.Child #4 had no address for one emergency contacts on record.
3.Child #5 had no addresses for either emergency contact on record.

Plan of Correction: Administrator will be going to Goodview to correct and organize the children's files to ensure they are up to date and have all the required documents inside.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that hazardous substances, shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1.The storage closet in the cafeteria was unlocked permitting access to cleaning chemicals to children. Children were observed going in and out of this storage space

Plan of Correction: Admin will have the custodians move their cleaning supplies to the back room away from the reach of the children.

Standard #: 8VAC20-780-510-P
Description: Based on medication review, the center failed to notify the parent of expired medication for pick-up within 14 days or the parent to renew the authorization.

Evidence: Child #6 had medication authorization that expired on 07/19/2023 on site during the inspection on 08/22/2023.

Plan of Correction: Admin will have a new medication authorization form filled out for the child and put with his medication.
