A unannounced monitoring inspection was conducted on August 25, 2023 beginning at 11:45am and ending at 12:30pm to investigate a self-reported incident regarding care, protection, and guidance. Staff interviews were conducted, and facility documentation was reviewed.
Violations have been cited related to information submitted by the facility of a report of an injury to a child while under the supervision of the facility that required outside medical treatment. Please see the Violation Notice for more details.

Standard #: 8VAC20-780-340-A Description: Based on staff interviews and documentation, facility staff failed to ensure the care, protection, and guidance of all the children enrolled in the program. Evidence: Staff #1 unhooked child A?s safety straps of the highchair before removing the tray. Due to Child A being unsecured in the seat when the tray was removed, he fell out of the highchair and forward on to the floor. The highchair?s seat height was 9 inches from the floor. Child A sustained a large bump on the front left side of his forehead that appeared immediately after the fall. It was confirmed through staff interviews that training was received regarding the appropriate procedure to remove a child from a highchair. Plan of Correction: Not available online.
