A self-report complaint was received in the licensing office on July 12, 2023 regarding four-year-old child rough handled by staff. On July22, 2023, on-site investigation was conducted. Interviews were conducted and observations were made.
Based on the preponderance of evidence gathered, violations were cited. For violation details, please refer to the Violation Notice.

Standard #: 8VAC20-780-40-E Description: Based on review of records and video camera footage, the center did not ensure that the center?s activities and services and facilities were maintained in compliance with required by standards.
Evidence:
1. On June 9, 2023, Staff #1 did not follow center?s discipline for children to new touch child with force in any way.
2. The center?s discipline policy requires redirection to guide the child to the appropriate behavior.Plan of Correction: 1. Staff person was immediately removed from classroom on Friday, June 9, 2023.
2. Termination of Staff member on Monday, June 12, 2023. This staff person violated the Kids `R? Kids standards as well as violated licensing standards.
3. Corporately reviewed licensing standards regarding forbidden actions at virtual KRK staff meeting on June 21, 2023.
4. Discipline Training for all staff on Tuesday, July 18, 2023 at July Staff Meeting
5. We are implementing a Head Count Sheet hourly- to be performed by admins throughout the day. We will use this to observe and identify and issues in real time.
6. We are evaluating all staff through the end of the year.
7. Each classroom (teachers in each class) will be observed weekly by admin and admin will complete a follow up to address any issues/behaviors. Additional training can and will be assigned as needed.
Standard #: 8VAC20-780-340-A Description: Based on interview and video footage, the center did not ensure that when staff were supervising children, they always ensure their care, protection, and guidance.
Evidence:
1. On June 9, 2023, during rest time in the Starfish classroom, Staff #1 grabbed Child #1 (4-year-old) by the left upper arm and forced child on mat.
2. On the same day, Staff #1 grabbed Child #2 (3-year-old) by the right upper arm and forced child on the mat.
3. Child #1 continuously kicked Staff#1 with his feet. Each time Staff #1 pushed child?s feet back toward the mat. Staff grabbed Child #1 by right leg and left ankle to put back on mat.
4. Staff #1 took the Child #1?s blanket and walked towards the trashcan. At the time, Administrator entered the classroom and Staff #1 left out the classroom. Before Staff #1 left out the classroom, Staff #1 threw Child #1?s blanket at the child.Plan of Correction: 1. Staff person was immediately removed from classroom on Friday, June 9, 2023.

Evidence:
1. Staff #1 rough handled Child #1 and Child #2 by grabbing the children by the upper arm and forced children to mat.
2. Staff #1 grabbed Child #2 by the right leg to turn over on the mat. Staff grabbed Child #1 by the left ankle to placed child back on the mat.Plan of Correction: 1. Staff person was immediately removed from classroom on Friday, June 9, 2023.
