All standards left undetermined will be review during the next unannounced monitoring inspection in approximately 2 months after the conditional license is approved and issued.

Technical Assistance:
Discussion that all undetermined standards and codes will be reviewed during the next unannounced inspection.

Discussion that the ownership will need to contact the subsidy vendor to continue this program when the license is issued.

Discussion that the small hole left by the hanging green leaf within the 2's-3's room will need to be patched and the blind cord located near the corner of the room will need to be raised. In addition, a bathroom floor cover located in the Full Day Montessori room will need to be re-screwed to the floor.

It was encouraged to have all staff purses locked-up.

Discussion of having spare crib sheets in reserve for parent that bring sheets that are either too large or too small for crib mattresses.

Comments:
This initial inspection was initiated by licensing staff and concluded on 08/31/2023, between the hours of 10 am to 12 pm. There were 41 children present, ranging from infant to preschool-age, with 9 classroom staff supervising, across 6 groups. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. Children records, and all staff records will be reviewed at the next unannounced monitoring inspection.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the center.

Violations:
Standard #: 8VAC20-780-500-B
Description: Based on observation, a changing pad used for diapering was no longer non-absorbent.

Evidence -
1. On the date of inspection (08/31/2023), a changing pad observed in the Infant II room showed visible signs of small rips all along both sides of the pad.
2. With the padding now visible through the various rips, it is now rendered absorbent and needs to be replaced.

Plan of Correction: A new pad has been ordered and should arrive next week. In the meantime, diaper changing will take place on the changing pad used by the Infant I class.

Description: Based on observation and interview, staff's personal medication was not kept in a locked place.

Evidence -
1. On the date of inspection (08/31/2023), in an unlocked, upper cabinet above the sink within the Full Day Montessori classroom, a bottle of medication was observed being stored.
2. When asked, the Licensing Inspection (LI) was told by classroom staff that the medication belonged to another staff person who had yet to arrive.
3. While a lock was attached to the cabinet, the lock itself was not secured to prevent access.
4. There were 14 preschool-age children within this classroom at the time of observation.

Plan of Correction: Corrected on the date of inspection.
