Technical Assistance:
Technical assistance was provided regarding staff records and background checks.

Comments:
A mandated monitoring inspection was conducted on August 7, 2023 from 12:00pm-1:30pm. There were 69 children in care with 10 staff supervising. Children were observed washing hands, preparing for lunch, watching a video, and eating lunch. Lunch served was spaghetti, broccoli, peaches and milk. Toddlers were observed sitting in a circle reading a book and infants were observed being fed and crawling freely.

A focused investigation was conducted in response to a self-reported incident regarding supervision of children. Based on interviews conducted and documentation reviewed, a violation was cited for care and protection of children and is identified in the Violation Notice

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review, each child did not have documentation of a physical examination by or under the direction of a physician.

Evidence: Child 1 and 2 did not have documentation of a physical examination.

Plan of Correction: Physicals have been requested but we will follow up.

Standard #: 8VAC20-780-70
Description: Based on record review, the center did not ensure all staff records were complete.

Evidence: Staff 1 was missing 2 documented references.

Plan of Correction: We will make sure to document all references.

Standard #: 8VAC20-780-80-A
Description: Based on record review, the center did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: In the young toddler's class, no children were marked present and there were 10 children present. In school age, seven children were marked present and there were 11 children in attendance. In primary 2, 12 children were marked present but there were 17 children in attendance.

Plan of Correction: We will make sure attendance is accurate.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure all hazardous items were kept in a locked place using a safe locking method.

Evidence: In preprimary 2, cleaning products were kept in an unlocked upper cabinet.

Plan of Correction: Cabinet was locked at time of inspection and all cabinets will be locked going forward.

Standard #: 8VAC20-780-340-A
Description: Based on interview, while supervising children a staff member did not ensure the care and protection of children.

Evidence:
Staff 2 and 5 stated on 8/4/2023 Staff 3 walked about 80 feet away from the 4 children they were supervising. While Staff 3 was talking to another teacher, Child 6 (two years old) pushed the fence and was able to crawl to the other side. Staff 3 and 5 made Staff 3 aware that Child 1 crawled to the other side of the fence and Staff 3 ran out to catch Child 6 who was running towards the parking lot.

Plan of Correction: The staff member no longer is employed by the program. The fence was repaired.

Standard #: 8VAC20-780-550-D
Description: Based on record review, the center did not implement a monthly evacuation drill.

Evidence: There was no July drill documented.

Plan of Correction: We will make sure to do and document our drills.

Standard #: 8VAC20-780-560-G
Description: Based on observation, the center did not ensure all requirements were met for food brought from home.

Evidence: In the school-age program, four lunch boxes brought from home were not labeled with names and dates.

Plan of Correction: Lunches will be labeled going forward.
