An investigation was initiated from 10:00 am - 11:00 am to investigate a self-reported incident involving a forbidden action and behavioral guidance. Interviews were conducted and video footage was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-400-B Description: Based on interviews and review of video footage it was determined that behavioral guidance was not always constructive in nature and age and stage appropriate. Evidence: On 08/17/2023 at approximately 4:00 pm the children in the two's class are seen sitting at the table for an art activity. Child A (Age two years) is seen running away from the table. Staff A is seen chasing after the child. Staff A gets down on the child's level and grabs them by the arms. Staff A then speaks to the child while putting their face close to the child's face in an intimidating manner, what was said was unable to be determined. Staff A then grabs the child by the left arm, lifting their legs off of the ground, places the child in the classroom bathroom and closes the door for over two minutes. Staff A enters the bathroom for less than one minute and then both staff A and child A are seen leaving the bathroom. It was unable to be determined what happened while staff A and child A were in the restroom. Staff A reported that they did not know why they placed child A in the restroom.
