Technical Assistance:
Discussed the children's physicals need to be conducted within the last 12 months. Discussed child immunization requirements/exemptions.

Discussed the written allergy list should include all children's allergies, sensitivities and dietary restrictions.

Discussed that any child for whom emergency medications have been prescribed must always be in care of a staff member who meets the medication administration training certification.

Discussed the TB requirements, staff qualifications and required training for new staff..

Comments:
An unannounced monitoring inspection was conducted on 9/07/2023 from 10:00am to 11:18am. At the time of entrance, 9 children were in care with 2 staff members present. Children were observed eating snack, washing hands, playing with legos and other toys and working on an art project. A selection of staff and children records, the physical space and evacuation drills were reviewed. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and staff were positive. Standard #: 8VAC20-780-160-A
Description: Based on record review, each staff member did not have documentation that the tuberculosis screening was completed within the last 30 days calendar days of the date of employment.
Evidence:
1. Staff #1 (start date 9/05/2023) had a tuberculosis screening dated 7/23/2023 and was completed more than 30 days of the date of employment.

Plan of Correction: TB screening documentation for staff #1 will be updated.

Standard #: 8VAC20-780-60-A-8
Description: Based on record review, the center did not obtain a written care plan for each child with a diagnosed food allergy to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1. Child #2 (start date 9/05/2023) had a diagnosed food allergy and the center did not have a written care plan to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: A written care plan signed by the physician will be obtained for each child with a diagnosed food allergy.

Description: Based on record review, staff records did not include all required information.
Evidence:
1. Staff #1 (start date 9/05/2023) did not have date of employment and documentation of two or more references before employment.

Plan of Correction: The first date of employment and documentation of two or more references will be filled out for the Staff #1.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation, the center failed to cover all unused outlets accessible to children under 5 years of age with child-resistant protective covers.
Evidence:
1. Two outlets were not covered with child-resistant protective covers by the medify air machine.
2. One outlet was not covered with child-resistant protective covers by the carbon monoxide detector.

Plan of Correction: All unused outlets will be covered with child-resistant protective covers.
