Comments:

An unannounced renewal inspection was conducted today. All groups were in ratio during the inspection. 5 staff records were reviewed. All were in compliance. Children were observed participating in a wide variety of activities, during diapering, washing hands, eating snack, preparing to transition to the outside, during circle time and playing in the gross motor room on the first floor. 5 child records, medications, and training records for on-going training were reviewed. See Violation Notice for non-compliances cited today. Emergency evacuation drills, emergency supplies, age and stage appropriate activities were all observed. Description:	Based on a review of records, one child record did not contain a completed physical within 30 days of enrollment. Evidence: Child #5?s record has no physical available for review. (Date of Enrollment: 7/1/22)
Plan of Correction:	Physical is in he student's records on site.
Standard #:	8VAC20-780-60-A
Description:	Repeat Violation Based on a review of records, child records do not include all required information. Evidence: 5 records reviewed. 1 of 5 records had no emergency contact information. (Child #3) 1 of 5 records was missing father?s work information. (Child # 4) 1 of 5 records had a parent listed as the second emergency contact. (Child #5)
Plan of Correction:	Child #3: outreach to parent to obtain emergency contact information. Child #4: Out]reach to parents to obtain father's work information - "father resides overseas" Child #5: Outreach to parent to obtain second emergency contact.

Description:	Based on a review of records, documentation of on-going training for staff, did not contain all required components. Evidence: Training certificates for staff did not contain documentation of the training delivery method.
Plan of Correction:	Implemented the state recommended forms for documenting training. Documentation of training now include delivery method.
