8VAC20-780-560G All food from home, including water bottles if filled at home, will need to be labeled with name and date.

Comments:
An unannounced renewal inspection was conducted on 09/07/2023 from 7:35am to 8:40am. There were 21 children ages 5 years to age 10 years old supervised by 3 staff. The physical plant, programming, attendance, menus, 3 staff records, 5 children?s records, 13 medications with corresponding authorization records, emergency drills, emergency supplies, and policies were reviewed. Children were observed participating in basketball, table games, art and snack. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-160-C
Description: Based on review of 3 staff records, the center did not obtain documentation of a negative tuberculosis (TB) test or screening for staff every 2 years from the date of the initial screening or testing.

Evidence: The record for Staff #1 did not have documentation of a negative TB test or screening withing 2 years of the last TB test. The most recent TB test on file was dated 8/17/21.

Plan of Correction: Negative TB test was provided on 9/9/23.

Standard #: 8VAC20-780-60-A
Description: Based on review of 5 child records, the Center did not ensure they obtain all of the required documentation for child records.

Evidence: The record for Child #1 did not contain the name, address and phone number of 2 designated people to call in an emergency if a parent cannot be reached. The record for Child #1 did not contain the father?s work place and work phone number.

Plan of Correction: Information was acquired and file is now complete.

Standard #: 8VAC20-780-510-E
Description: Based on review of 13 medications and medication authorizations, the center did not ensure they follow the policy for administration of medication.

Evidence:
The medications for Child #4 and Child #5 did not contain parent authorization or authorization from the child?s physician.

Plan of Correction: Proper documentation was provided to a parent for a 10 day only form and doctors appt will be soon.

Standard #: 8VAC20-780-560-J
Description: Based on observation, the center did not ensure that tables were sanitized before and after each use for feeding.

Evidence: The table where 2 children were eating their snack was not sanitized prior to the children eating as required.

Plan of Correction: Cleaning procedures were reviewed with all staff.
