Comments:
An unannounced monitoring inspection was conducted on August 9 2023 between 8:45am and 10:30am. There were 67 children in the direct care of 12 staff members. Children were observed playing outside, washing hands, being diapered, being fed, and working on projects. A review of children's records, staffing records, the evacuation plan, and the physical plant of the center was reviewed and inspected. A citation is listed on the violation notice. Description: Based on record review, the center did not ensure that a follow-up tuberculosis screening was conducted for each staff member every two years from the date of the initial screening.

Evidence: Staff 3 had a tuberculosis screening with a date of June 24, 2021 and does not have documentation of a follow-up screening on file.

Plan of Correction: Not available online. Contact Inspector for more information.
