Comments:
Today an unannounced Monitoring Inspection took place between the hours of 11:10am and 1:40pm. There were 2 classrooms observed with a total of 16 children with 4 staff within the supervision guidelines. The children were observed playing freely, completing worksheets, transitioning from outside play to eating lunch and naptime. A complete inspection of the physical plant, children and staff records, fire drill log and emergency procedures were observed during this inspection. There was an adequate number of staff with current certification in CPR and First Aid. Staff-to-child ratios were in compliance. Areas of non-compliance are identified on the Violation Notice.

Standard #: 22.1-289.035-A
Description: Based on record review and interview, all employees did not undergo repeat background checks every five years.
Evidence: Staff #1's most recent documented fingerprint results are dated 5/8/2018. Staff #1 confirmed that they had not yet completed repeat fingerprints.

Plan of Correction: I can get it done next week.

Standard #: 8VAC20-780-130-A
Description: Based on record review, the center did not obtain documentation that each child has received the immunizations before the child can attend the center.
Evidence: Child #1 (date of enrollment: 8/23/2023) record does not contain documentation of immunizations.

Plan of Correction: I've asked the parents for this paperwork. I'll stay more on top of them.

Standard #: 8VAC20-780-130-E
Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child #4 (23 months) most recent immunizations are dated from when the child was 12 months old.

Plan of Correction: I've asked the parents for this paperwork. I'll stay more on top of them.

Standard #: 8VAC20-780-140-A
Description: Based on record review, the center has not obtained a physical exam for each child within the first 30 days of enrollment.
Evidence: Child #3 (date of enrollment: 7/24/2023) does not contain a physical exam.

Plan of Correction: I've asked the parents for this paperwork. I'll stay more on top of them.

Standard #: 8VAC20-780-160-A
Description: Based on record review, each staff member did not submit documentation of a negative tuberculosis (TB) screening that has been completed no more than 30 days prior to the first date of employment.
Evidence: Staff #4 (date of hire: 4/3/2023) TB screening is dated 1/20/2023.

Plan of Correction: I'll start checking the test dates.

Standard #: 8VAC20-780-60-A
Description: Based on record review, children's records did not contain all required information.
Evidence: Child #1's record does not contain documentation that the center had verified the child's age and identity.

Plan of Correction: The parents don't have the birth certificate yet. I've let them know they can bring the letter from the hospital.

Standard #: 8VAC20-780-80-A
Description: Based on observation and record review, the center did not maintain accurate daily attendance for each group of children.
Evidence: At the start of the inspection, 15 children were observed in care. 11 children were documented as arrived at the center.

Plan of Correction: The parents should be signing in on the app and the teachers should be signing in on the clipboards. I'll remind the staff again to sign the attendance sheets when children arrive.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials, were not kept in a locked place using a safe locking method that prevents access by children.
Evidence: Several spray cleaning agents were observed in the cabinet under the changing table in the infant classroom. The cabinet door is not locked.

Plan of Correction: I will remind the staff again about locking cleaners.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation electrical outlets did not have protective covers.
Evidence:
1. Two uncovered outlets were observed at the entrance of the center next to the children's cubbies.
2. Two uncovered outlets were observed next to the sink in the children's bathroom.

Plan of Correction: I'll put them on and remind the staff.

Standard #: 8VAC20-780-380-A
Description: Based on interview, a daily scheduled is not posted.
Evidence: When asked about the daily schedule, the center director confirmed that there was no schedule posted.

Plan of Correction: We have a schedule but it must have gotten taken down. I'll post it.

Standard #: 8VAC20-780-450-A
Description: Based on observation, each child did not have linens consisting of a top and bottom cover.
Evidence: Three children were observed napping without a top cover.

Plan of Correction: I will email the parents to remind them that they need to bring blankets to the center and I'll keep them here.

Standard #: 8VAC20-780-550-D
Description: Based on record review and interview, the center did not implement monthly practice evacuation drills.
Evidence: There was no evacuation drill documented for August 2023. The program director confirmed that a drill was not practiced in August.

Plan of Correction: We'll start putting reminders on our calendars to do them monthly.

Standard #: 8VAC20-780-570-B
Description: Based on observation, infants who could not hold their own bottles were not held when fed.
Evidence:
1. One infant was observed laying in an activity chair while staff #5 held the bottle in the child's mouth while holding another child in their lap.
2. One infant was observed laying in an activity chair with a bottle propped up with a blanket. The bottle was observed falling away from the child.

Plan of Correction: Corrected during inspection.
