Comments:
An unannounced monitoring inspection conducted on September 7, 2023. There were a total of 31 children with 3 staff members supervising. Children were observed engaged in various age-appropriate activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, children and staff records, special care and emergencies and nutrition. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-780-70
Description: Based on record review and interview, the center failed to maintain a staff record for each staff person to include the name, address, verification of age requirement, job title and date of employment or volunteering, name, address, and telephone number of a person to be notified in an emergency which shall be kept at the center.
Evidence:
1. There was no information on file for Staff #3 to review.
2. Staff #1 confirmed there was no information on file Staff #3.

Plan of Correction: I will make sure the rover desk informs the substitute staff to bring their emergency contact information.

Standard #: 8VAC20-780-240-A
Description: Based on record review and interview, the Virginia Department of Education-sponsored orientation course was not completed within 90 calendar days of employment.
Evidence:
1. Staff #3 stated he had not completed the VDOE orientation course.
2. There was no file to review Staff #3 training certificates.

Plan of Correction: I will ensure I tell the rover desk to ensure the substitute staff have all their training certificates with them when they come to the program.

Standard #: 8VAC20-780-245-J
Description: Based on medication review and staff interview, the center failed to ensure that for any child for whom emergency medications (such as epinephrine auto injector)have been prescribed shall always be in the care of a staff member who meets the medication administration training.
Evidence:
1. Staff #1 acknowledged there was not a MAT trained staff on sight at the time of the inspection.Staff #1 stated the two MAT trained staff members were absent.
2. Child #5 has a Medication/allergy plan for emergency administration of epinephrine auto injector. The inspector confirmed the medication was on sight.
3. Child #6 has a medication/allergy plan for emergency administration of epinephrine auto injector and hypoglycemia. The inspector confirmed the medication was on sight.

Plan of Correction: I registered 2 additional staff for training and should have the training completed by 12/31/23. Also, I will inform the Rover Desk to ensure they send us a MAT trained substitute.
