Technical Assistance:

Technical assistance was offered related to the following: 1. Supervision of toddlers when using the playground. 2. Staff records. 3. Emergency drills and plan

Comments:

An announced initial inspection was conducted on 9/11/23 from10:00 am to 12:40 pm. There were no children present on site. The physical plant, staff qualifications, background checks, policies and procedures, first aid supplies and the emergency preparedness plan were reviewed. 5 staff records were also reviewed. Classrooms were measured to determine the center's capacity. The classrooms are equipped with supplies, equipment, and age appropriate materials for children. Infant cribs were inspected to assess for crib compliance. The outdoor playground equipment and surface was inspected. Many areas related to the supervision and care of children could not be inspected due to no children in care at this time. Areas of non-compliance are listed in the violation notice. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website. Standard #:	8VAC20-780-160-A
Description:	Based on record review, tuberculosis (TB) screening for a staff member did not contain complete information. Evidence: Staff # 1's TB screening did include the physician's signature.
Plan of Correction:	The TB screening has been dropped off to be signed.
Standard #:	8VAC20-780-70
Description:	Based on records review, written references for staff members did not contain complete information. Evidence: 1. Reference checks for five staff members were missing the signature of the person that completed the reference checks. 2. Reference checks for two of five staff members was missing the following information: the name of the person contacted and the date of contact.
Plan of Correction:	The references were completed in their entirety and a signature block has been added for the forms going forward.

Description:	Based on records review, the emergency preparedness plan did not contain complete procedural components. Evidence: The evacuation, shelter in place and lockdown plans were missing the procedures to reunite children with parents or an authorized person to pick up the child.
Plan of Correction:	The reunification plans have been added to the plan.
