An unannounced monitoring inspection was conducted on 8/31/2023 from 3:00 pm to 6:30 pm. There were 14 children in care at the beginning of the inspection, ages six years to 12 years, in one group under the care of six staff. The Licensing Inspector reviewed three children?s records, four staff records, and other program records. The children were observed doing homework and quiet indoor play.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff record review, the center failed to ensure staff had TB screening done at the time of employment and prior to staff being in contact with children.
EVIDENCE:
Staff 3 has a documented hire date of 4/2/2021 and was observed with children during the inspection. The record for Staff 3 does not have documentation that a TB screening was done.
