Comments:

A subsidy health and safety inspection (SHSI) was conducted on September 7, 2023 from approximately 11:25 am to 2:30 pm. There were a total of 16 children present, ranging in ages from 2 to 4 years, with 5 staff supervising. A total of 5 children records and 4 staff records were reviewed. During the inspection, children were observed during rest time and playing with toys. Staff were observed having positive interactions with the children. The center is equipped with toys and supplies and items were available to the children. A licensing inspection was conducted on the same date as the subsidy supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description:	Based on staff records review and interview, the vendor did not ensure that each staff complete annually the Virginia Annual Health and Safety Update Course sponsored by the Department. Evidence: 1. The record for Staff #2 (date of employment: 5/17/2021) contained documentation of the Virginia Annual Health and Safety Update Course dated 6/15/2021. There was no documentation that the course has been completed since 6/15/2021. 2. The record for Staff #3 (date of employment: 3/23/2015) contained documentation of the Virginia Annual Health and Safety Update Course dated 7/15/2015. There was no documentation that the course has been completed since 7/15/2021.
