An unannounced inspection was conducted on September 6, 2023 in response to a complaint received on 08/31/2023 regarding allegations in the area of special care provisions and emergencies. The inspector was onsite from approximately 9:45 am to 1:15 pm.
There were a total of 42 children present, ranging in ages from 15 months to 5 years, with 12 staff supervising. A total of 1 child record was reviewed and three staff were interviewed.
The preponderance of the evidence gathered during the investigation supports noncompliance in the area of procedures for emergencies, the complaint is determined to be valid.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not
acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Standard #: 8VAC20-780-550-P Complaint related: Yes Description: Based on record review, the child day center did not maintain a written record of children's injuries that contained all required information.
Evidence:
1.The record for Child #1 documented incident reports dated 8/21/23 and 8/23/23.
2.The 8/23/23 incident report did not contain the date and time that the parent was notified of the injury and documented the signature of one staff. A second staff signature or parent signature was required.
3. The 8/21/23 and 8/23/23 incident reports did not document future action to prevent recurrence of the injuries.Plan of Correction: Not available online. Contact Inspector for more information.
