Comments:
An unannounced monitoring inspection conducted on September 14, 2023 between the hours of 11:00am and 1:00pm. There was a total of 35 children with 8 staff members supervising. Children were observed during eating lunch, washing hands, outside activity, bathroom and handwashing procedures, and resting and/or sleeping. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, children and staff records, special care and emergencies and nutrition. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-780-40-G
Description: Based on record review and interview with the center director, it was determined that the center did not obtain proof of child's identity and age requirements as stated in the Code of Virginia.
Evidence: 1. The record of Child #1 did not have documentation for proof of the child's identity and age.
2. The center director confirmed the proof of identity of Child #1 was not obtained.

Plan of Correction: I will contact the parents today and tell them to bring in the proof of identity.

Standard #: 8VAC20-780-40-M
Description: Based on observation and interview, the center failed to maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities and dietary restrictions documented in the allergy plan. This list was not kept in each room or area where children are present.
Evidence:
1. The allergy list was not posted in any classroom.
2. The center director acknowledged the list had been taken down and was not posted at the time of the inspection.

Plan of Correction: The list was taken down to laminate but the machine broke. I will post the list immediately in each classroom.

Violation corrected during the inspection.
