An unannounced non mandated inspection was conducted on 08/28/2023. There were 20 children, ages 2 months-4 years, and 5 staff members present during the inspection. A total of 5 children?s records and 1 staff record, were reviewed. The inspectors arrived at 2:50 PM and departed at 4 PM.

Standard #: 22.1-289.035-A Description: Based on record review of one staff person, the center failed to ensure that
required background checks are repeated every 5 years per the Code of Virginia.
Evidence:
Staff # 1 had a fingerprint eligibility letter dated 07/18/2018. An updated fingerprint
eligibility letter was not provided.Plan of Correction: The staff person and administration stated that they would schedule the finger print appointment ASAP.
Standard #: 8VAC20-780-140-A Description: 5th REPEAT VIOLATION
Based on record review and interviews, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance.
Evidence:
