Comments:
An unannounced renewal inspection was conducted on 8/7/23 from 9:10 a.m. to 12:43 p.m. with the director. There were 57 children in care, ranging in age from three months to 12-years-old, supervised by 10 staff. The children were observed preparing for a field trip and playing outside. Five child records and five staff records were reviewed. Three medications and authorization forms were reviewed and the center has six staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery operated radio were observed. The most recent Fire Inspection on file was dated 7/29/23 and the most recent Health Inspection on file was dated 1/18/23. Lunch served was Pasta, smoked sausage, cheese stick, bread, fruit, and milk. Violations:
Standard #: 22.1-289.036-A
Description: Based on record review and interview with director, there was no documentation of a current central registry search, national fingerprint background check and sworn disclosure statement for the applicant.

Evidence: The central registry search was dated 12/15/17, the national fingerprint background check was dated 3/15/18, and the sworn disclosure statement was dated 2/22/18 for the applicant, repeat background checks are required every five years.

Plan of Correction: Was submitted the day of inspection. We have not received the CRS background only

Description: Based on review of five staff records, the center did not obtain documentation of the results of a central registry search within 30 days of employment for all staff members.

Evidence: The record for Staff #1 with a start date of 5/24/22, the results of the central registry search background check was dated 6/29/22.

Description: Based on interview with the director, the center did not update injury prevention procedures annually.

Evidence: There was no documentation of injury prevention procedures. Director confirmed that documented injuries had not been reviewed and a prevention plan was not updated.

Description: Based on interview with the director, the center did not ensure they maintained a written record of daily attendance for each group of children documenting arrival and departure of each child in care as it occurred.

Evidence: On 8/7/23 from 9:00 a.m. until the end of the inspection at 12:43 p.m. there was no written record of daily attendance for any group of children in care.

Plan of Correction: The teachers were told to keep a manual list. This day the printer and phone lines were down

Description: Based on a review of six staff files, the facility did not ensure that all program leaders had documentation of high school program completion or equivalent on file

Evidence: The record for Staff # 3 and Staff # 5, with a title of teacher, did not contain documentation of education.

Standard #: 8VAC20-780-270-A
Description: Repeat Violation

Based on observation, it was determined that not all areas of the center inside and outside were maintained in a clean, safe condition

Evidence:
1. In the Overflow classroom a child?s table was placed upside down on top of another table, creating a tripping hazard. The location of the table was at the entrance of the classroom and accessible to children, the table had a broken leg and was not able to stand upright.

2. In the Overflow classroom there were two, square, metal dry erase boards, approximately 24? x 18?, laying against a shelf of toys in the children?s play area. The dry erase boards had metal corners and rounded metal installation hardware protruding upward creating an impalement and scratch hazard.

3. Peeling paint was observed throughout the Noah?s Ark Room (ages six weeks to 24 months), Gift of Spirit (16-24 months), Fruit of the Spirit 1 (ages two to three-years-old), Fruit of the Spirit 2 (ages two-years-old) and the Songbirds (ages four and five-years-old) classrooms.

4. In Noah?s Ark Room, children ages six weeks -16 months, an adult size glider rocker with no cushion was observed. The upholstery springs, used to support the cushion, and the metal bearings at the base of the rocker were exposed creating an impalement, pinch and scratch hazard. During the inspection, Child #4 climbed on top of the exposed upholstery springs of the glider rocker.

5. In the Songbirds classroom, the bathroom had screws protruding upward from the base of both toilets. Toilet #1, had a screw that extended approximately 15 thread lengths on one side and a a screw that extended approximately seven thread lengths on the other side of the base of the toilet. Toilet #2, had a screw that extended approximately 15 thread lengths on one side and approximately 22 thread lengths on the other side of the base of the toilet. Toilet #2 was not secured to the floor, the base of the toilet was slid slightly off to the left, exposing the ghost image of where the correct location of the toilet should be.

6. Tape was covering a large crack in a window pane of the School Age classroom.

7. On the older playground two wooden boards, approximately 18? in height with semi-circle cut outs at the top, from a previous water table that has been removed, protruding from the ground. The wood had two peaks on each board from the cut-outs, the wood was splintering and disintegrating, creating an impalement and scratch hazard. An enclosed wooden structure, used to store water hoses, had approximately six rusted nails protruding, and a rusted metal hinge, approximately 1.5? length, sticking out of the base of the structure creating an impalement and scratch hazard.

8. On the school age playground a tree that had fallen in the wooded area along the perimeter of the school age playground, the tree was leaning over the fence and the branches of the fallen tree were hanging into the playground area were the basketball court was located. A section of the fence and the branches were taped off with caution tape. At the time of the inspection approximately eight children and one staff member were observed playing basketball within several feet of the hanging braches of fallen tree.

9. On the younger playground a plastic rat trap, with a round opening approximately 2.5? to 3? in diameter, was observed in an area that children have access.

Description: Repeat Violation

Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the Overflow classroom, the unlocked cabinet along the back wall, contained three bottles of toilet cleaner and pre-moistened sanitizing wipes, all had warning labels stating, ?Keep out of Reach of Children? and ?Caution?.

2. In the Fruit of the Spirit 2?s classroom (two-years-olds) a door that could not be fully shut, leading to the unoccupied kitchen, contained a gallon of bleach on the floor with a warning label stating, ?Keep out of Reach of Children? and ?Warning?.

Description: Based on observation and interview, behavioral guidance was not constructive in nature, age and stage appropriate and intended to redirect children to appropriate behavior.

Evidence: At lunch time In the Fruit of the Spirit class (two-years-old), while waiting for lunch Child #6 was strapped into a toddler chair and lifted up a home living chair next to them, causing the chair they were in to tip over. Staff reported that the child was strapped into the chair in order to prevent the child from running around the classroom during lunch.

Plan of Correction: Has been corrected the bleach and other cleaning items placed in a locked cabinet

Standard #: 8VAC20-780-410-2
Description: Based on observation and interview, the center did not ensure that a forbidden action was not utilized with children in that a child was enclosed in a space that they could not freely exit themself.

Evidence: At lunch time In the Fruit of the Spirit class (two-years-old) Child #6 was observed sitting away from the group in a toddler chair with the strap fastened, a small table in front of them and two cots positioned on either side of the child. Staff stated this is how the child eats meals to prevent them from running around the classroom during meals.

Plan of Correction: The chair was removed from the class. The teacher is aware that she is not supposed to have the chair

Description: Based on review of medications, the center failed to follow medication requirements that all prescription medications were maintained in the original, labeled container.

Evidence: The medication for Child#7 was in a labeled plastic baggie and was not in the original, labeled container and the medication had expired on 4/23.

Description: Based on review of medications, the facility failed to have written authorization from the parent to administer the medication as required.
