Comments:
An unannounced monitoring inspection was started in the center on September 18, 2023 from 3:30 PM until 5:50 PM and concluded on September 19, 2023. At the time of entrance, 39 children were in care with 9 staff members present. The sample size consisted of 5 children?s records and 4 staff records. Children were observed preparing for their afternoon routine and playing outside.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-50-B
Description: Based on documentation review and staff interviews, the licensee did not ensure records and reports on children and staff required shall be maintained and made accessible for two years after termination of services or separation from employment unless specified otherwise.

Evidence: Child #6 and Child #7 withdrew from the center on 9/1/2023 and their records were not available for review as confirmed by the Center Director.

Standard #: 8VAC20-780-80-A
Description: Based on documentation review and staff interviews, the licensee did not ensure that for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: There were 6 separate groups of children observed and the following groups did not have attendance sheets indicating the children?s arrival and departure.
1.Mirrors II (older infants) room had an attendance sheet listing 10 children and only 8 were observed in care. The departure of 2 children had not been documented.
2. The following classrooms were observed playing separately outside and there was not an attendance list for each group of children who were being supervised.
a. Windows I ages 2-3
b. Windows II ages 2 ? to 3
c. Ladders I ages 3 to 4
d. Ladders ages 3 to 4
