Comments:

A renewal inspection was conducted on 09/20/2023. There were a total of 8 children with 1 staff members present. Children were observed during outside learning time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description:	Based on a review of staff records, the CDC did not ensure all employees had repeat background checks every five years. Evidence: Staff #3 did not have results of a current central registry search on record during the inspection.

Description:	Based on a review of staff records, the CDC did not ensure all employees had a TB test or screening at least every two years from the date of the first initial screening or testing. Evidence: Staff #2 had TB results dated 6/19/2021 (expired 6/19/2023) Staff #3 did not have TB results in their record.

Description:	Based on a review of records, the CDC did not ensure program leaders had required documentation to be in the program leader role. Evidence: Staff in the extended day program did not have documentation of program leader qualifications.

Description:	Based on a review of staff records, the CDC did not ensure orientation was documented prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. Evidence: Staff #4 (date of hire 5/27/23) and staff #5 (date of hire 5/15/2022) did not have orientation documented in their record.
