Comments:
The inspector conducted an unannounced mandated inspection on 8/29/2019 from 10:45 a.m. until 1:15 and on 09/05/2019 from 2:00 until 2:45. There were 110 infants and children in care with staff supervising at all times. Infants were observed being fed and playing. Children were
observed playing inside and outside, begin read to, washing hands, eating lunch and resting. Ten children?s records and ten staff records were reviewed. Medications and the corresponding paperwork was reviewed. The violation from the last inspection had been corrected. Areas on
non-compliance are identified on the violation notice. Please contact your inspector at 276-698-9981 with any questions.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center was not maintained in a clean and safe environment.
Evidence:
1. The inspector observed paint peeling on the railing in the orange room and blue room.
2. The inspector observed a playhouse on the playground that had an interior area around the
window that was broken and splintered.

Plan of Correction: Maintenance will be notified. The house will be removed.

Description: Based on observation, the center was not maintained in a clean and safe environment.
Evidence:
1. The inspector observed paint peeling on the railing in the orange room and blue room.
2. The inspector observed a playhouse on the playground that had an interior area around the
window that was broken and splintered.

Plan of Correction: The inspector observed and measured resilient surfacing that was not maintained at the depth of 6 inches.
Evidence:
1. The inspector observed the area around the blue and purple comets and the yellow swings that had a depth of mulch between 1-3 inches.

Description: Based on observation, the center had mediation on site where the authorization had
expired and it had been longer than 14 days.
Evidence:
1. The inspector observed two medications on the lower level that the authorizations had
expired and it had been longer than 14 days.

Plan of Correction: Staff removed the medications and was going to dispose of them.
