A renewal inspection was conducted on 09/20/2023. There were 18 children present and 6 staff. A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed 3 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-A Description: Based on record review the center did not ensure all staff had the required repeat background checks.
Evidence:
The last documented fingerprinting for Staff #2 (hired 04/04/05) was dated 08/06/2018 and has expired.Plan of Correction: This was an oversight. Appointments have been made with filed print to have fingerprinting completed as soon as possible.

Evidence:
Staff #1, hired on 09/05/2023 did not have documentation of fingerprinting. Staff #1 was observed working with the children during the inspection.Plan of Correction: This was an oversight. Appointments have been made with filed print to have fingerprinting completed as soon as possible.
Standard #: 8VAC20-780-70 Description: Based on record review the center did not ensure all requirements were kept for each staff person.
Evidence:
Staff #1, hired on 09/02/2020 did not have documentation of having at least 2 references checked prior to employment.Plan of Correction: Two references will be obtained and verified for staff #1 this week and documented in the file.
Standard #: 8VAC20-780-270-A Description: Based on observation the center did not ensure all areas and equipment inside and outside were maintained in a safe condition.
Evidence:
1. The screws on top and on the sides of the swing play structure had more than 2 threads exposed, posing an entanglement hazard.
2. In the toddler room there were small hard pieces of pasta, jacks, buttons, and sticks used for a lesson. These objects posed a choking hazard to the children in that room under the age of 2 years-old.Plan of Correction: The small objects were removed from the classroom and replaced with more age appropriate objects for that age group.
Standard #: 8VAC20-780-330-B Description: Based on observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence:
There was approximately 2 inches of resilient surfacing in the fall zones of the Little Tykes slide on the toddler playground and the school-age rock climber. The fall zones of the rock climber also had weeds and grass growing through the surfacing.Plan of Correction: Mulch will be replenished and maintained in all fall zones of play equipment.
