Comments:
A complaint inspection was conducted on 09/14/2023 in response to a complaint received on 09/12/2023 alleging staff on duty alone with the children. The inspector arrived at 7:20 AM and departed at 10:00 AM. There were 4 children present, ranging in ages from 20 months - school age. The investigation included observation, interviews and review of documentation. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision and special care and emergencies.

Information gathered during the investigation determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Based upon observation, the facility has not ensured that areas and equipment are maintained to be safe.
Evidence:
Upon arrival for the inspection, there was a 20 month old child (child 1) in care in the front room just inside from the front door. There were toys and equipment for older children in this room, such as multi-colored counting bears, that presented choking hazards for a 20 month old..

Description: Based upon observation and staff interview, the facility has not ensured that during the stated hours of operation, there is always on the premises when one or more children are present one staff member who meets the qualifications of a program leader and an immediately available staff member.
Evidence:
1. The inspector arrived at the facility at 7:20 AM as three children were being dropped off into care. Staff 1 was in the facility alone with child 1. These four children remained in care with staff 1 until a second staff person arrived at 7:55 AM. No other staff were present in the building during this time.
2. Staff interview confirmed that there have been times when there was one person on duty in the facility while the second staff person took children to or picked up children from public school.
3. The facility was unable to provide staff time cards to indicate that at least two staff are on duty at all times while children are in care.

Description: Based upon observation, the facility has not ensured that children's hands are washed after eating.
Evidence:
1. Three children arrived with breakfast from home. The children's hands were not washed after eating.
2. The children were observed eating the facility provided breakfast. Two children got up from the breakfast table and began playing with toys before washing their hands.

Description: Based upon observation, the facility has not ensured that tables are sanitized after being used for eating.
Evidence:
The children were observed having breakfast. Following breakfast, staff 3 wiped the tables with a wet rag from the kitchen. The tables were not sanitized using a disinfecting agent.
