Technical Assistance:
Consultation provided on Lockdown drills
Reviewed standard 330B regarding resilient surfacing and fall zones.
Discussed the need to make sure at least two staff are in Building B when one or more children are present, this is in addition to maintaining ratios at all times.
Because parents can drop off and pick up in Building B you need to make sure all of the required posted information in conspicuous in Building B.

Comments:
An unannounced renewal inspection was conducted on 9/18/23 with a lead teacher from 1pm to 3:15 pm. There were 19 children present with 4 staff providing supervision. The ages of the children ranged from 13 months to 4 years. The children were napping during the walkthrough.
There were no medications on site. Five staff and six children's files were reviewed during this inspection. The Fire inspection was conducted on 8/3/23 and the Health inspection was conducted on 8/17/23. A follow up inspection was conducted on 9/20/23 to measure Building B, which is new space the center wants to open and use for daycare.The areas of non compliance for the 9/18/23 inspection is outlined on the violation notice.

Violations:
Standard #: 8VAC20-780-80-A
Description: Based on a review of the attendance log, it was determined that the a written record of daily attendance that documents arrival and departure of each child in care was not maintained in the two year old classroom.
Evidence:
There were 4 children present in the two year old room however the attendance had no children present.

Plan of Correction: Reviewed attendance requirements and expectations with staff.

Standard #: 8VAC20-780-270-A
Description: Based on observation, it was determined that areas and equipment outside was not maintained in a safe condition.
Evidence:
On the infant/toddler and the preschool playgrounds there were protruding bolts more than 2 threads on the chain link fence and gates.

Plan of Correction: We will make sure to cut all screws to ensure they are no more than 2 threads. When cut we will make sure there are no sharp edges.

Standard #: 8VAC20-780-330-B
Description: Based on observation and measurements, it was determined that the playground failed to maintain at least 6 inches of resilient surfacing and failed to maintain at least 6 feet of fall zone around play equipment.
Evidence:
1. On the infant/toddler playground there were two tan and green portable slides as well as a pink/green and blue composite climber and slide sitting on bare ground and not on at least 6 inches of resilient surfacing as required. There were toys and a fence all within 6 feet of the pink/green and blue composite climber and slide. The standard requires at least 6 feet of clearance where no object is located in the fall zone.
2. On the Preschool playground there was 1/2 inch of pea gravel surrounding the Blue/red and yellow composite structure with two blue slides . The standard requires at least 6 inches of pea gravel surrounding this structure.
There was no pea gravel 12 inches in front of the red tunnel slide. The tunnel slide requires at least 6 inches of resilient surfacing.

Plan of Correction: We will remove the portable slides and p lace the other structure on the younger playground off limits until we make more long term plans.
We will rake pea gravel in the fall zones on the older playground.
We will ensure at least 6 feet clearance for all playground structures that children can fall from or with moving parts.
