Comments:
An unannounced monitoring inspection was conducted on September 19, 2023. The inspection began at approximately 4:20 p.m. and concluded at 5:30 p.m. There were 103 children present, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 9 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: Based on record review and interview, the center failed to ensure that all employees shall undergo background checks every five years.
Evidence:
1. The record for Staff 2 contained a criminal background check dated 6/19/2018 and sworn statement dated 6/16/2018. Staff 2 should have undergone a criminal background check before 6/9/2023 and completed and undated central registry before 6/16/2018.
2. The record for Staff 7 contained a central registry background dated 7/16/2018. Staff 7 should have undergone a central registry background before 7/16/2023.
